0 0 <br /> SAIC - Engineering and Infrastructure <br /> Health and Safety Plan for Chevron EMC Field Operations <br /> [Job Name: Tracy Byron Road Project—Additional Soil and Groundwater Investigation <br /> Emergency Evacuation <br /> Emergency evacuation of the site may be necessary under certain conditions. In the event that the site must be <br /> evacuated, the SHSO or other personnel will invoke a "Stop Work"authorization. One long blast from a portable <br /> air horn or vehicle horn will serve as the site emergency alarm. All work is to stop immediately. The SHSO or <br /> FM will call 911, utility company(if necessary), and the PM. Workers will exit the site and proceed to the <br /> designated primary assembly point.A secondary assembly point shall be identified in the event the primary <br /> assembly point is unsafe. The primary assembly point will be southwest corner of South Lammers Road and <br /> West Byron Road. The secondary assembly point will be the south west corner of Von Sosten and West Byron <br /> Road. The SHSO or FM will use the SAIC Tailgate Safety Form to call roll and account for on-site personnel. If <br /> anyone is missing, that person's name will be forwarded to emergency response personnel. Emergency <br /> evacuation practice drills will be performed on the first day of work at the site and at least once every 3 to 6 <br /> months for each major task. Evacuation practice drills will be documented on the Emergency Evacuation <br /> Practice Drill Report. <br /> In the event that an alarm or other emergency action occurs at an adjacent facility, the SSHO will evaluate the <br /> emergency and reference the wind direction to determine whether or not the field team should assemble at the <br /> designated rally point(s)for the project. If the designated rally point(s)are deemed to be unsafe, the SSHO will <br /> have the field team meet at an alternate location and conduct an accountability of all field personnel. When all <br /> field personnel have been assembled, the SSHO will lead the field team away from the emergency in a safe <br /> direction and conduct emergency notification protocols when the team has been relocated to a safe area. <br /> The SHSO or FM will use the SAIC Tailgate Safety Form to call roll and account for on-site personnel. If anyone <br /> is missing, that person's name will be forwarded to emergency response personnel. Emergency evacuation <br /> practice drills will be performed during the first week of work at the site and at least once every 3 to 6 months for <br /> each major task. Evacuation practice drills will be documented on the Emergency Evacuation Practice Drill <br /> Report. <br /> The SSHO will contact safety representatives and inquire about the emergency alarms and response protocols <br /> used at the facility prior to field mobilization. The SSHO will also verify that the emergency alarm to be used <br /> onsite does not conflict with an alarm used at adjacent facilities. The SSHO will present this information to the <br /> field team during daily tailgate safety briefings. The SSHO will include the telephone numbers of the safety <br /> departments at adjacent facilities in his list of emergency contacts in the event that an onsite emergency could <br /> affect an adjacent facility. <br /> Emergency Communication <br /> See Site Safety and Health Hierarchy Organization Chart included in this HASP.
