The EHD i <br /> required all three monitoring wells to be installed and limited the well <br /> screen length to twenty feet at most. Later work plan revision modified <br /> MW-2, MW-3, and MW4 to dual casing, nested wells. <br /> I <br /> 05/27/1998 Date monitoring wells MW-2s/d, M -3s/d, and MW-4s/d were installed. <br /> 10/21/1998 Date of Foothill's Report of Findings of Phase 2 Site Investigation that <br /> details the installation of three dual cased, nested monitoring wells. <br /> 04/27/2001 Date of Foothill's work plan Expansion of Phase 2 Investigation to the <br /> East which proposed the installation of one off-site, dual-cased <br /> monitoring well, MW-5a and MW-51). <br /> 11/08/2001 Date monitoring well MW-5a anc MW-5b were installed. Details in <br /> Foothill's Report of Findings M415sld Installation, dated 1-2-2002. <br /> Monitoring well MW-5a later changed to MW-5S (shallow) and monitoring <br /> well MW-5b changed to MW-51D (deep). <br /> 02/23/2005 Date of Foothill's Site Conceptual Afodel (SCM). <br /> 04/13/2005 Date of the EHD letter commenting on SCM that required newly <br /> discovered domestic well on-site to be destroyed and MW-1 to be <br /> destroyed and replaced so as not to provide a vertical pathway across <br /> two water bearing zones. The EHD denies a request to grant a No <br /> Further Action status to the site since the 1,2-DCA plume is not fully <br /> characterized and requires a work plan to continue the investigation. <br /> 06/10/05 On-site `domestic' well destroyed under witness and permit issued by the <br /> EHD. <br /> 07/19/05 Meeting held with the responsible party and consultant at the EHD at the <br /> request of consultant. Concerns included costs, 1,2 DCA and PCE <br /> investigation, and if from UST, and CUF reimbursement. <br /> 09/16/05 Date of EHD work plan comment and approval letter for CPT -1, -2, & -3 <br /> installations sent. <br /> 10/17/05 Date of EHD letter commenting and denying Foothill's October 5, 2005 <br /> Work Plan for MW-1S, MW-9D, MW-6S and MW-6D installations and <br /> MW-1 destruction as inadequate. Addendum needed. <br /> 10/21/2005 Date Cone Penetrometer Test (CP ) borings CPT-1 & 2 were installed. <br /> 11/08/05 Date of EHD approval of Foothill's Work Plan for MW-9S and D, and <br /> MW-6S and D (addendum) dated October 29, 2005, for monitoring well <br /> installations and existing monitoring well MW-1 destruction. <br />
