Adequate and necessary elements in the Workplan are the installation of 2 <br /> Monitoring Wells(MWs),soil and groundwater sample collection and analysis, and a proposed feasibility <br /> study. The Workplan is approved with the following conditions: <br /> I. The screened intervals for the proposed MWs 5 and 6 must be appropriate for the suspected <br /> contaminant lithological pathway. MW screens should be designed to extend above the groundwater <br /> level to allow for relevant sample collection. MWs currently located at this site are screened below the <br /> groundwater level and may not reflect actual groundwater conditions in regards to contamination. It <br /> may be necessary to construct additional MWs to evaluate groundwater near the source area and to <br /> intercept contaminants migrating off site. <br /> 2. Contact PHS/EHD for an inspection appointment prior to performing feasibility testing and MW <br /> construction. <br /> 3. Encroachment permit(s) and boring pen-nit application(s) concurrent with $89.00 permit fee(s) should <br /> be submitted to PHS/EHD by April 20,2000. <br /> 4.
