California St. <br /> Stockton, CA 95204 <br /> San Joaquin County Public Health Services Environmental Health Division(PHS/EHD) <br /> approved a Site Investigation Work Plan on April 22, 1999 and an Over Excavation <br /> Addendum Plan on May 7, 1999 prepared by EMCON for the above referenced site. The <br /> work outlined in these Work Plans to further investigate contamination at the site has not <br /> occurred. The work as outlined in the approved Work Plan must be completed within 90 <br /> days of the date of this letter. If the work is not completed within that time frame the <br /> above referenced site will not be in compliance with PHS/EHD directives. <br /> A well permit application and $89.00 permit fee should be submitted to PHS/EHD as <br /> soon as possible. After the permit has been issued the work can be scheduled.
