iN R+1 <br /> Ground Water Monitoring Well Installation Report <br /> FLAG CITY CHEVRON <br /> 6421 Capitol Avenue, Lodi, California <br /> 1.0. INTRODUCTION <br /> Advanced GeoEnvironmental,Inc (AGE)has prepared this well installation report for 6421 Capitol <br /> Avenue,Lodi, California(the site),in accordance with guidelines issued by the California Regional <br /> Water Quality Control Board(RWQCB)-Central Valley Region for investigations of underground <br /> storage tank(UST)sites The report documents procedures and results for the installation of one off- <br /> site, dual-completed ground water monitoring well at 6440 West Banner Avenue (McDonald's <br /> Restaurant) for the purpose of assessing the extent of methyl tertiary butyl ether (MTBE) ground <br /> water impact north of the Flag City Chevron site The location of the site is illustrated in Figure 1, <br /> general features at the site are illustrated in Figure 2 Background information and a description of <br /> prior investigations at the site were included in AGE's Final Remediation Plan, dated 31 August <br /> 2000 <br /> The following is a brief summary of events for the November-December 2004 well installation <br /> activities performed at the site <br /> • 30 November 2004 - Dual-completed ground water monitoring well MW-20 was installed <br /> to 50 feet below surface grade(bsg), soil samples were collected at five foot intervals during <br /> drilling <br /> 07 December 2004 - Well development activities performed at well MW-20 <br /> • 12 January 2005 -Elevation and Global Positioning System(GPS) survey of well MW-20 <br /> 2.0. DRILLING AND SAMPLING PROCEDURES <br /> On 30 November 2004,the dual-completed ground water monitoring well MW-20 was installed at <br /> the site, the soil boring for MW-20 was advanced vertically to a maximum depth of 50 feet bsg <br /> utilizing a CME-75 high torque,truck-mounted drill rig equipped with 8-inch diameter hollow-stem <br /> augers All soil cuttings generated from drilling activities were containerized in properly labeled <br /> Department of Transportation(DOT)-approved model 17H 5 5-gallon drums located in the northeast <br /> section of the site,monitoring well locations are depicted in Figure 2 Ground water monitoring well <br /> installation procedures for the site were outlined in Work Plan Addendum - April 2004, dated 14 <br /> April 2004, prepared by AGE and approved by the RWQCB by letter dated 06 May 2004 <br /> (Appendix A) <br /> Advanced GeoEnvironmental,Inc <br />
