INTRODUCTION <br /> At the request of Mr Richard Bokides of the Bokides Family Limited Partnership, Advanced <br /> GeoEnvironumental, Inc (AGE) has prepared thus Final Remediation Plan(FRP) for 6421 Capitol <br /> Avenue, Lodi, California(the site) The site setting is illustrated on Figure 1 Site structures and well <br /> Ilocations are depicted on Figure 2 <br /> The purpose of this FRP is to detail the design, installation and operation of an on-site ground water <br /> extraction and treatment system Ground water remediation(pump and treat) will consist of ground <br /> water extraction in conjunction with treatment utilizing granulated activated charcoal (GAC), <br /> MTBE-hydroscopic synthesized filter media technology The FRP is prepared as required by <br /> I California Regional Water Quality Control Board-Central Valley Region (RWQCB) Cleanup and <br /> Abatement Order No 5-00-700, dated 01 March 2000 and is in accordance with RWQCB guidelines <br /> Appendix A Reports Tri-Regional Board Staff Recommendations for Preliminary Investigation and <br /> Evaluation of Underground Tank Sites <br /> S2.0. BACKGROUND <br /> The site is located at the northeast intersection of Capitol Avenue and Thornton Road, approximately <br /> 500 feet south of California State Highway 12 and approximately 5 miles west of Lodi, California <br /> (Figure 1) The site is located in the northeast quarter of Section 14, Township 3 North, Range 5 <br /> East, Mount Diablo Base and Meridian (Teminous Quadrangle, 7 5-Minute USGS Topographic <br /> Series, 1978, photo revised 1993) <br /> 21 SITE DESCRIPTION <br /> The property is currently utilized as a Chevron fueling facility and mini-market The site consists of <br /> one building structure, six fuel dispensers,three 12,000-gallon gasoline USTs and one 12,000-gallon <br /> diesel UST (Figure 2) Soil and ground water on the site has been impacted by petroleum <br /> hydrocarbons as the result of an unauthorized release from a UST formerly located on the site <br /> 2 2 PREVIOUS INVESTIGATION <br /> The following is a summary of previous investigation at the site as the result an unauthorized release <br /> of fuel products to the subsurface <br /> • 02& 03 May 1991-AGE was informed by the San Joaquin County Public Health Services - <br /> Advanced GeoEnvironmeutai,Inc <br />
