INTRODUCTION <br /> In accordance with the request of Mr Richard Bokides of Flag City Chevron, Advanced <br /> GeoEnvironmental,Inc (AGE)has prepared this Closure Summary Report(CSR)summarizing all <br /> field activities conducted at 6421 Capitol Avenue(formerly 6421 Paddock Place), Lodi, California <br /> (the site) Based upon the results of subsurface investigation of the underground storage tank(UST) <br /> and fuel dispenser areas at the site, it is AGE's opinion that Cleanup and Abatement Order(C&A) <br /> No 5-00-700 be rescinded and that no further investigative or corrective action be required(NFAR) <br /> for this facility The site setting is illustrated on Figure 1, the underground storage tank and fuel <br /> dispenser locations, site building structures and existing monitoring well locations are depicted on <br /> Figures 2 and 3 <br /> This CSR has been prepared in accordance with the California Regional Water Quality Control <br /> Board(RWQCB) - Central Valley Region guidelines (Tri-Regional Board Staff Recommendations <br /> For Preliminary Investigations and Evaluation of Underground Tank Sites) for No Further Action <br /> Requests (See Appendix A) <br /> �l <br /> 2.0. BACKGROUND <br /> The Flag City Chevron facility (the site) is located at the northeast intersection of Capitol Avenue <br /> and Thornton Road,approximately 500 feet south of California State Highway 12 and approximately <br /> 5 miles west of Lodi, California (Figure 1), the site is further located in the northeast quarter of <br /> Section 14, Township 3 North, Range 5 East, Mount Diablo Base and Meridian (Teminous <br /> Quadrangle,7 5-Minute USGS Topographic Series, 1978,photo revised 1993),in a topographically <br /> flat area at an elevation between 5 and 10 feet above mean sea level The San Joaquin County Tax <br /> Assessor's Office lists the site's boundaries as Parcels 23 and 24 on Map 32 in Book 55 (APN 55- <br /> 32-23124) <br /> The Flag City Chevron property is currently owned by the Bokides Family Limited Trust (Mr <br /> Richard Bokides) Mr Bokides has stated that he leased the property from Mr Cecil Dillon between <br /> 1984 and 1994, and he purchased the property circa 1994 The property has been utilized as a retail <br /> fueling station from approximately 1984 to present day, prior to 1984, the property was utilized as <br /> a saddle shop Mr Bokides was a part of the Flag City Chevron day-to-day business operations <br /> between 1984 and 1997, however, the UST's have been operated by Flag City First Investment <br /> (Nick and Karen Dhingsa) from 1997 to present day The Flag City Chevron property currently <br /> consists of one building structure(mini-mart),six fuel dispensers,three 12,000-gallon gasoline US Ts <br /> and one 12,000-gallon diesel UST (Figure 2) <br /> .� An unauthorized release(UAR)of miscellaneous motor vehicle fuel was reported by the San Joaquin <br /> Advanced GeoEnvironmental,Inc <br />
