i <br /> . 31 August 2005 <br /> AGE-NC Project No 96-0232 <br /> Page 2 of 30 <br /> County Environmental Health Department(SJCEHD)in May 1993 during repairs and/or upgrades <br /> to the UST vent line system, as a result, the SJCEHD directed that a subsurface investigation be <br /> performed beneath the UST area A copy of the UAR is in Appendix B <br /> In a letter dated 16 February 2000, the RWQCB notified all responsible parties for the Flag City <br /> Chevron facility(Bokides Family Ltd Partnership,First Investment Flag City Inc and Flag City Ltd <br /> Partnership)that the RWQCB would assume the regulatory lead from the SJCEHD due to the high <br /> MT'BE concentrations detected at the site and due to the MTBE detections from the Rocky's <br /> Restaurant domestic water well In order to expedite the investigation and cleanup of the MTBE <br /> plume, the RWQCB issued Cleanup and Abatement Order No 5-00-700, dated 01 March 2000 <br /> (Appendix C) <br /> There are currently two, separate ongoing leaking underground storage tank(LUST) investigations <br /> being performed near the Flag City Chevron site The first LUST site is identified as the Three B's <br /> Truck Plaza(14749 North Thornton Road)and is located approximately 200 feet west-northwest of <br /> the Flag City Chevron site (Figure 3), the Three B's Truck Plaza facility is utilized for commercial <br /> and retail fueling, and general commercial truck repairs and is under a SJCEHD directive to <br /> investigate former leaking USTs that were removed from the site circa 1998 The second LUST site <br /> is identified as New West Petroleum(6437 West Banner Avenue)and is located approximately 300 <br /> feet north-northeast of the Flag City Chevron site (Figure 2), New West Petroleum is utilized for <br /> retail fueling and is under a SJCEHD directive to investigate petroleum-impacted ground water <br /> identified on Banner Avenue <br /> 21 PRELIMINARY SITE ASSESSMENT <br /> It is our understanding that due to a potential change in the property and business ownership,three <br /> ground water monitoring wells (MW-1, MW-2 and MW-3)were installed at Flag City Chevron on <br /> 02 and 03 May 1991 near the former UST and fuel dispensing areas as part of a preliminary <br /> environmental site assessment All three wells were drilled to a depth of 24 feet below surface grade <br /> (bsg), and completed with perforated casing at depths between 9 and 24 feet bsg Soil samples <br /> collected at depths of 5 feet and 15 feet from all three wells during drilling,none of the analyzed soil <br /> samples detected any petroleum hydrocarbon constituents of concern <br /> Between 1991 and 1993, seven ground water monitoring events were performed at the site on wells <br /> MW-1,MW-2 and MW-3 Generally,groundwater levels were encountered at depths between 9 and <br /> 13 feet bsg, ground water flow direction was typically reported towards the northwest at gradients <br /> of 0 002 ft/ft or less Dissolved petroleum hydrocarbons were not detected during the initial May <br /> 1991 ground water samples, however benzene was subsequently detected from well MW-1 during <br /> the May 1993 event at a concentration of 16 micrograms per liter(µg/1)and from well MW-3 during <br /> Advanced GeoEnvironmental,Inc <br />
