Please note that the Cumulative Table of Well Data and Analytical Results
contained an error in the results reported for the sample collected on February 24, 1995 from MW6;
actually, 16 ppb xylene was detected rather than 2.4 ppb which was reported.
In correspondence dated September 6, 1994, PHS/EHD requested that a work plan be submitted to
complete the groundwater investigation. PHS/EHD received a work plan to continue the investigation
dated October 16, 1994 and requested verbally that permitting to obtain access be initiated. On February
28, 1995, 1 met on site with you and Jason Fedota (GTI) to discuss possible locations for additional soil
borings and monitoring wells. On August 8, 1995, 1 was informed by you that Caltrans had informally
agreed to allow placement of an off site monitoring well. You indicated at that time that you would verify
that GTI had submitted a permit application to Caltrans. On September 19, 1995, 1 notified you that in
order to remain in regulatory compliance, permitting should be completed and field work should be
scheduled by October 30, 1995.
PHS/EHD has been notified by Caltrans that an encroachment permit has been issued; therefore, PHS/EHD
drilling permit applications and associated fees should be submitted within 14 days of the date of this letter
and drilling scheduled within 30 days of the date of this letter. Should you fail to respond to these
compliance dates, please be informed that regulatory enforcement actions will be initiated.
if you have any questions or to schedule a site inspection, please contact Mary Meays at (209) 468-0337.
Donna Heran, REHS, Director
Environmental Health Division
Mary Meays, Senior REHS Diane M.
