PHS/EHD has prepared the following comments for your consideration and response. <br /> The Addendum failed to respond to PHS/EHD's directive that the addendum include a scope of work necessary to <br /> evaluate remedial alternatives,but did propose an additional soil boring to be placed on site. However,a map was not <br /> included in the addendum indicating where this soil boring was to be located. The description of the proposed <br /> relocation of monitoring well MW8 indicated that it would be located approximately 140 feet further downgradient <br /> (southeast)of the formerly proposed location which was 100 feet south of MW7 and 140 feet southwest of MW8. <br /> Regarding the historical record of compliance which Chevron has demonstrated at this site,it should be noted that the <br /> last quarterly report which PHS/EHD has received was the"Second Quarter 1996 Groundwater Monitoring"report <br /> which included the results of the May 7, 1996 sampling event. PHS/EHD has a difficulty with understanding how the <br /> historical record of compliance can be deemed appropriate and timely. <br /> Given the size of the plume,the concentrations in wells properly screened in and above first encountered groundwater, <br /> the remaining hydrocarbon contaminated soil which is located below the first encountered groundwater,it is likely that <br /> some active remedial efforts will be necessary. <br /> Please submit a PHS/EHD drilling permit,a Caltrans or City of Stockton encroachment permit(depending on the <br /> ownership of the of parking lot located near the northeast comer of South Center and Lafayette Streets)and the$89 <br /> permit fee. Once the PHS/EHD permit is issued,please contact PHS/EHD at least two working days prior to the <br /> scheduled field activities,so that PHS/EHD can schedule a field inspection. Also,you must contact PHS/EHD <br /> following the field activities to discuss the selection of soil samples to undergo laboratory analysis. <br /> The report of results shall include a summary of all the analytical soil and groundwater results which have been <br /> obtained for the site and the results of the well survey. The report should be submitted within 45 days following the <br /> completion of field activities. Also,the third and fourth quarterly reports should be submitted immediately. <br /> PHS/EHD had postponed final review of the proposed addendum in hopes of reviewing the most recent quarterly <br /> sampling results.
