For TA0170807 (Tank 1, 87 slave) and <br /> TA0170809 (Tank 3, 87 siphon), beneath "Pipe Monitoring is Performed Using the Following Method(s)"change <br /> "Yes" to "No" for"Electron Line Leak Detector Performs 3 GPH Leak Test." For TA0170808 (Tank 2, Master) and <br /> TA0170810 (Tank 4, premium unleaded) beneath "Pipe Monitoring is Performed Using the Following Method(s)" <br /> enter"Yes" for"ELLD Triggers Automatic Pump Shutdown" and "Yes"for"ELLD Failure/Disconnect Triggers <br /> Automatic Shutdown." <br /> The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log <br /> into CERS, make the necessary changes, and submit for review by the EHD. <br /> This is a repeat violation, Class Il. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> A cold start was documented by the designated operator on October 21, 2016. The designated operator noted in <br /> the 10/28/16, 11/28/16, 12/30/16, 1,25/17 and 2/28/17 designated operator monthly inspection reports that upon <br /> arrival, the Veeder Root console was showing sudden loss alarms on T1, T2, T3 and T4. No alarm history was <br /> available. The designated operator noted all alarm histories were Iosg'due to a cold start done by the contractors <br /> working during dispenser replacements. The designated operator indicated sensors, TLMs and PLLDs need to be <br /> tested for"operability" tests. The designated operator stated that the owner needs to have the issues corrected <br /> "ASAP." <br /> Upon arriving for the monitoring system certification on 3/27/17, it was noted that the sudden loss alarm was <br /> documented in the panel printout for T1, T2, T3 and T4 for March 2017, January 2017 and December 2016. <br /> There was a work order on 3/9/2017 addressing the sudden loss alarm, however, there was a sudden loss alarm for <br /> T2 Blue Master on 3/13/2017 indicating the sudden loss alarm has not been properly addressed. Immediately have <br /> a properly licensed trained and certified contractor address the alarms. <br /> This is a minor violation. <br /> 206 CCR 2712 Failure to comply with UST operating permit conditions. <br /> A permit to inspect the conversion frames and piping leaks for the four dispensers installed in October 2016 was not <br /> submitted to the Environmental Health Department(EHD)for approval.
