The most <br /> current training was completed on February 29, 2016 for one employee and the four remaining employees were last <br /> trained in 2015. The designated operator shall train facility employees for which he or she is responsible in the <br /> proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not <br /> limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> This is a repeat violation, Class II. <br /> 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> Approximately two quarts of liquid (possibly product)were found in the under dispenser containment for dispenser <br /> 1/2. Seepage was observed above the shear valve in two locations. All primary containment for the UST system <br /> must be tight. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed <br /> component . <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> The annual monitor system certification (MSC) testing was not completed on March 27, 2017. The service <br /> technician indicated that the testing could not be completed because the electronic line leak detectors were not <br /> wired correctly to do positive shutdown. None of the sensors was tested in the submersible turbine pump <br /> (STP) sumps, fill sumps and under dispenser containment (UDC) sumps. The annular space sensors, overfill <br /> protection by automatic tank gauge (ATG) , power out, sensor out and line leak detectors were not tested <br /> today. The fill sumps were not visible to determine if there were liquid in the fill sumps and if the sensors were <br /> properly positioned. The four spill containers were tested today during the inspection. Please note that the <br /> owner of the facility will be charged a fee of$139 per hour to complete the MSC. <br /> A checklist, addressing the violations found to date, was provided to the owner on the day of the MSC inspection <br /> Note: The 87-master and 91-product spill containers did not contain a capacity of 5 gallons. When the <br /> 87-master and 91-product spill containers fail, the spill containers shall be replaced with a spill container that <br /> has a minimum capacity of 5 gallons, is protected from galvanic corrosion, and has either a drain valve, which <br /> allows drainage of the collected spill into the primary container, or provides a means to keep the spill containers <br /> empty pursuant to CCR, Title 22, section 2635 (b) (1). <br /> FA0003619 PR0231708 SCO01 03/27/2017 <br /> EHD 23-01 Rev.09/22/16 Page 5 of 6 Underground Storage Tank Program OIR <br />
