ATC SITE SAFETY PLAN <br /> Job Name: Guaranty Bank—Channel Street <br /> Job Location: 221 East Channel Street, Stockton, California <br /> Client Name: Mr. Michael Saffell <br /> Project Number: 54.05257.5406 <br /> General Work Description: Site Investigation <br /> SSP Prepared by: Jim Kundert <br /> On-site Safety Officer: Jim Kundert <br /> Project Safety Officer: Jeanne Homsey <br /> This site health and safety plan (SSP) must be kept on-site and accessible to all on-site <br /> employees during all phases of site operations. The SSP must be reviewed and signed by all <br /> on-site employees, subcontractors, and visitors prior to entering the site. General safe work <br /> practices are identified in the ATC Associates Inc.'s Injury and Illness Prevention Manual <br /> and apply to all ATC work sites. <br /> The provisions set-forth in this SSP shall apply to ATC employees and any subcontractors <br /> working for ATC at the job site. All personnel working for ATC at the job site must read this <br /> SSP, and sign the attached Compliance Agreement before entering the work area. Field <br /> personnel may deviate from the safety provisions set-forth in this SSP, but only to upgrade or <br /> increase the safety requirements. ATC personnel may suspend work if unsafe working <br /> conditions are identified or observed, or unauthorized modifications to the safety provisions <br /> set-forth in this SSP are made. If changes in site or working conditions require changes in <br /> safety procedures, appropriate amendments to this SSP will be provided by the ATC project <br /> manager or supervisor,with the written approval of a ATC project safety officer or the ATC <br /> Health and Safety Director. <br /> PAGE 3 <br />
