Contact the local fire department, local planning or building department and the San <br />Joaquin Valley Air Pollution Control District for any applicable permits. <br />2. Any changes shall be submitted to the San Joaquin County Environmental Health <br />Department (EHD) for approval prior to commencing work. All new addenda supersede <br />the previous ones. <br />3. If any fuel is to remain in the underground storage tank, it must be continuously <br />monitored. <br />4. All components must be certified by an independent testing organization. (UL, ULC, <br />ANSI, API, ASTM, etc. See California Code of Regulations (CCR), Title 23 Chapter 16 <br />Table B for complete list). The component(s) must bear the appropriate mark as <br />required by the independent testing organization. <br />5. Only 100% diesel is permitted for storage in this UST at this time. <br />6. Submit an updated Unified Program Consolidated form for USTs (Form B) and monitoring <br />plan within 30 days of the completion of work. <br />7. Secondary containment testing must be performed on the diesel UST and all product <br />lines. Test results must be received by this office within 7 days of completion of work. <br />8. Complete and submit a copy of a UST Installation Certificate of Compliance (Former <br />Form C) no later than 30 days after work is completed. <br />9. Qualified service technicians and installers must possess or work under the direct and <br />personal supervision of an individual physically present at the work site who possesses all <br />of the following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), <br />and <br />b. All valid, current California UST certificates issued by the International Code <br />Council (ICC), that are applicable to the work being performed, and <br />Page 1 of 2 <br />
