The map submitted on CERS doesn not include the <br />dispenser sensors. A site plan must be submitted identifying the locations where monitoring will be performed. <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload <br />a copy of the UST Monitoring Site Plan. <br />This is a repeat violation, Class II. <br />204 <br />CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br />An approved, current copy of the monitoring plans were not available for inspection. A copy of these plans shall be <br />accessible on site at all times. Immediately locate and retain a copy on site. <br />This is a minor violation. <br />319 <br />CCR 2635(b) No spill bucket, failed to contain five gallons, or failed to provide a means to keep it empty. <br />The 87, 91, diesel spill containers failed to have a minimum capacity of five gallons. All spill containers shall have a <br />minimum capacity of five gallons. Immediately contact a properly licensed, trained, and certified contractor to repair <br />or replace the spill container under permit and inspection of the EHD. <br />This is a minor violation. <br />Overall Inspection Comments: <br />An inspection checklist was provided to the facility operator on the day of inspection. The EHD has written the <br />complete report which replaces the initial checklist. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the complete inspection report. <br />Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br />documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br />Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br />applicable), and the Owner Statement of Designated Operator Compliance. The UST Owner/Operator: Written <br />Agreement, if applicable, can be stored at the facility or uploaded into CERS. <br />Documents reviewed: CERS submittals, Designated operator (DO) monthly inspection reports, DO training log, <br />Maintenance records, Alarm logs, Test results, Alarm history <br />Documents provided: Return to Compliance Certification form <br />Page 4 of 5 <br />
