Ms Lori Duncan Project No 880-039 lA <br /> June 18, 2003 <br /> Page 4 <br /> 1 The fieldwork was performed in accordance with a site-specific Health and Safety Plan <br /> prepared by Shaw in general accordance with 29 CFR 1910 120 and Title 8, California <br /> Code of Regulations, Section 5192 The Health and Safety Plan included safety <br /> procedures for work to be performed at the site, chemical hazard information, site safety <br /> officers, and preferred medical emergency locations <br /> Installation of One On-Site Dual Completion Well <br /> On March 24, 2003, one dual completion well MW-4R (S&D) was drilled on-site at the <br /> location shown on Figure 2 Cascade Drilling (C57# 717510) drilled the well boring $ <br />' using hollow-stem auger techniques The boring was continuously cored from <br /> 15 feet bgs to the total depth of 80 feet bgs to provide a complete lithologic description <br /> of subsurface conditions Both wells are constructed of 2-inch-diameter Schedule 40 <br /> PVC casing and 0 020-inch machine-slotted well screen One well (MW-4RD) is <br /> screened from approximately 75 to 80 feet bgs The annular space around the screen is <br /> packed with RMC Pacific #3 sand to approximately 73 feet bgs A 22-foot seal of <br /> bentonite pellets was placed above the sandpack A second well (MW-4RS) was <br /> rinstalled with a screened interval from 35 to 50 feet bgs The annular space around the <br /> screen is packed with RMC Pacific #3 sand filter pack from approximately 33 to 51 feet <br /> bgs Above the sandpack is a 2-foot seal of hydrated bentonite chips The well boring <br /> was than filled with neat cement grout to a depth of 1 5 feet bgs The top of the well is <br /> protected by a traffic-rated vault box and locking well cap The vault box is installed <br /> flush with the ground surface, and set in concrete <br /> Seven soil samples from the well boring were submitted to a laboratory for the analysis <br /> of TPHg by EPA Method 8015B, and benzene, toluene, ethylbenzene, and xylenes <br /> (BTEX), ds-isopropyl ether (DIPE), ethyl tert-butyl ether (ETBE), MtBE, tert-amyl <br /> methyl ether (TAME), tert-butyl alcohol (TBA), 1,2-dichloroethane (1,2-DCA), and <br /> ethylene dibromide (EDB) by EPA Method 8260B Drilling and sampling procedures <br /> are included in Attachment B <br /> ASACRFP1\COMMONIsacDP120031PgV8018800391A Sae doc <br />
