<br /> Ground water monitoring well MW-10 will be constructed of 4-inch-diameter,flash-threaded,Sckedule
<br /> 40 polyvinyl-chloride(PVC)pipe and screen. The well will be completed with 0.02 inch slot width well
<br /> screen with a scr,.ened interval extending from 30 to 45 feet below grade. The screened interval will
<br /># intersect the water table from 5 feet above the water table and extend to 10 feet below the water table
<br /> allowing for seasonal ground water fluctuations. The annular space in the well will he riled with No.3
<br /> Lonestar and from 1 foot above the screened In to the total depth of the well. A 2-foot-thick
<br /> bentonite seal will be placed above the sand pack in the annular space of the well. Above this, the
<br /> annular space in the well will be filled with cement grout containing 5 percent bentonite. A well
<br /> ccnstruction diagram is enclosed illustrating well specifications.
<br /> After installation,the monitoring well will be developed,purged,sampled,and surveyed. Ground water
<br /> samples will be subwitted to a state-cert;ied laboratory for analysis of BTEX and TPH as gasoline.
<br /> Please find enclosed the necessary ground w.-ler monitoring well permit application and required fee
<br /> of 589.00.
