Both of these work plans were responded to by the EHD in letter dated <br /> June 18, 2008, and included the approval to install MW-10. In January 2009, pertaining to the aforementioned work <br /> plans, the EHD issued a permit for the installations of MW-5, MW-6, and MW-11. <br /> I did not see any documentation of MW-10 being rescinded in our staff notes from the time the work plans were <br /> responded to and when the installation permits were issued.
