The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames. The designated operator failed to check and note all required items on the visual inspection reports. -The March 2018 inspection report included an incorrect monitoring certification test date. -The 11/8/18 L2 (87 annular) high liquid alarm was not documented. -Section V&VI were left blank in the 10/29/18, 12/18/18, and 2/19/19 inspection reports. -Section V&VI were filled out by the designated operator instead of the owner/operator in the 11/20/18 and 1/22/19 inspection reports. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation. 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. On 6/27/18 an L2 (87 annular) high liquid alarm occurred. On 7/18/18 the alarm was found by the designated operator during the monthly inspection. On 7/27/18 the alarm was stopped and corrected by the designated operator/technician. The interstitial space shall be monitored with a continuous monitoring system connected to an audible and visual alarm system. The monitoring system shall be monitored by the site in order to detect alarms at the earliest opportunity and alarms shall be responded to by the owner/operator immediately when found. Ensure the UST system is being continuously monitored and all alarms are addressed in a timely manner. This is a repeat violation, Class II. 307 CCR 2631(g), 2632(c) Monitoring system of the interstitial space of a double walled system not maintained.
