Only one shallow monitoring well (MW-11) will be installed instead of the four <br /> proposed. <br /> 2. Intermediate wells MW-110 and MW-111 will be installed as proposed; wells MW-108 <br /> and MW-109 will be moved west of their proposed locations (approximately half the <br /> distance to wells MW-5 and MW-6) as overhead utilities and trees allow. <br /> 3. A deep well MW-202 will be installed near MW-2. The new well will be developed and <br /> sampled on the day of installation with a 24-hour laboratory turn around time. If the <br /> well is non-detect, then two additional deep wells (MW-205 & MW206) will be <br /> installed near wells MW-5 & MW-6. If MW-2 is impacted, then three additional deep <br /> wells (MW-208, MW-209 & MW-210) will be installed near wells MW-108, MW-109 <br /> &MW-110. <br /> 4. The deep wells will be screened in the 70-80 feet below grade surface interval. <br /> 5. Develop the new monitoring wells. <br /> 6. Place the new and existing wells on a quarterly monitoring schedule for a period of one <br /> year. <br /> 7. Prepare a site characterization report after well installation. <br /> 8. Prepare four groundwater monitoring reports. <br /> Listed below is a proposed schedule for implementing this project. <br /> Proposed Bidding of Project <br /> To remain in compliance with the USTCFP, Geological Technics Inc. requested a bid from <br /> V&W Drilling to drill and install up to nine groundwater monitoring wells (one shallow, four <br /> intermediate and four deep). GTI has found V&W to be very cost effective for three primary <br /> reasons: <br /> • reasonable charge rate <br /> • fast drilling equipment <br /> • well trained crew <br /> These costs have been included in the attached opinion of cost spreadsheet prepared giving <br /> estimated costs for performing the work approved by SJC PHS/EHD. The opinion of cost <br /> spreadsheet shows itemized cost for the tasks proposed. In an attempt to make this Pre- <br /> Approval flow smoothly, GTI has used the same unit costs for this work that has previously <br /> been approved by the USTCFP for other projects. <br /> Geological Technics Inc. accepts field support services and lab invoices and adds a 15% <br /> overhead fee. GTI then pays these charges. <br /> Tentative Schedule for Implementing Work <br /> Task Description Schedule <br /> • Prepare USTCFP Cost Pre-Approval for work December 2002 <br /> • Receive approval of costs from USTCFP January 2003 <br /> • Drill, install &develop the 9 MWs February 2003 <br /> • Perform a groundwater monitoring event including new wells March 2003 <br /> • Prepare a Site Characterization Report&Quarterly Monitoring Report April 2003 <br />
