EMPLOYEE TRAINING FORM <br /> TRAINING TOPIC: See attached hand outs <br /> HMMP/CERS/SPCC Plan for 2019. This document/program has been updated. <br /> HMMP/Hazardous Materials Management Plan. And review of the GHS. Globally <br /> Harmonized System This plan is reviewed annually, to insure that all employee(s) <br /> understand and know of the following. Spill Control and Counter Measure Plan. Portions <br /> of the plan are reviewed with employees, including and not limited to the Plan <br /> Procedures. A. Hazardous materials that are on site. (Right to Know) B. How to report a <br /> spill, and how,or what to do in the event of a spill. (Instructions are in the Contingency <br /> Plan). Review. <br /> 1. MSDS sheets, or now known as SDS sheets. Safety Data Sheets. Where to go for <br /> information and or what is required by the employer to the employee. Employees are <br /> given training annually in regards to the HMMP. This is mandatory as the program may <br /> change due to new chemicals and or old chemicals being deleted from its inventory. <br /> Employees are trained on how to read SDS sheets. The sheets can be complicated to read, <br /> and it is the Safety Director who will go over the SDS sheets so that all employees may <br /> better understand them. Where they are located, and what to do if one is missing. Who to <br /> contact: Contacting the immediate supervisor as well as the Safety Director so that the <br /> document can be re-printed, and or a chemical can be properly identified by all. The plan <br /> is reviewed in regards to the site/facility map. Where to go and how to respond in the <br /> event of a major spill and or fire. In the event of such emergency and or need: Each <br /> manager and or lead may assume the responsibility to perform a head count and give this <br /> information to the emergency responders. Medical and emergency contacts: Documents <br /> are posted and given to employees as to emergency numbers and local hospital <br /> information. All employees have the right to contact the emergency services should they <br /> feel the need.No employee is to fight a fire or respond to an emergency if it is beyond <br /> their capabilities. However, if an employee voluntarily assists and or responds to an <br /> emergency of his/her free will, the employee is to put on and have on all his/her PPE <br /> personal protective equipment when responding and or attending to a need. PPE,Personal <br /> Protective Equipment(PPE): Employees are given PPE at time of hire and are given new <br /> PPE should the employee-need it. Hazard Identification: Employees are trained to inform <br /> the immediate supervisor and or the Safety Director should the employee find/identify a <br /> hazard. This is to prevent an injury, death and or spill. Fire extinguisher training is <br /> performed annually; no employee is to help in a fire and or spill that may cause injury or <br /> death. All employees are trained to perform a self-assessment of any injuries and dangers. <br /> Call 911 for all emergencies that are beyond their capabilities to handle. House cleaning <br /> Keep isles and workplaces clear and free of hazards, examples, extension cord on the <br /> ground can be a trip hazard,hot torch next to a paper source, can be a fire hazard,remove <br /> and or place things,tools and equipment away properly so as to prevent hazards. Isles are <br /> to be kept clear, so people can move about without hindrance. Place floor sweep and or <br /> pads on spills and leaks, to reduce the risk of a fall and or slip. See hand out for more info <br /> and items covered. Maintenance of machinery: is to be performed regularly. This helps <br /> eliminate potential spills and or hazards'. Contingency Plan. This plan has been updated <br /> and reviewed. Employees who are hired are given instruction to review the plan and if <br /> they see and or require a change in the plan they to notify the Safety Director and it will <br /> be reviewed with Management and if needed it will be amended.
