for any electrical work and no employee shall be directed to wire anything they are not <br /> trained to do, leave this to a professional electrician. As for all items which do affect the <br /> employee, such as when working and or performing tasks on equipment, trucks, and or <br /> vehicles, all employees must place a lock out tag on the equipment. No employee other <br /> than the employee who originally placed this tag may remove it. No employee may horse <br /> play and or activate a vehicle without first assure the tag is removed,the"all clear"is <br /> given, to assure no person is still in harm's way, and only then, may the equipment be <br /> activated. Anyone found bypassing the system may be terminated. Employees working <br /> on equipment, shall not ask and or have another employee to activate and or start a piece <br /> of machinery without first"clearing himself from harm first". Stand away, get out of the <br /> way of any machinery. Example given. Employees are reminded, that the tools that they <br /> use, need to be inspected and replaced if damaged. Red tag and or destroy. All guards <br /> shall be in place and no triggers and or safety locks shall be tampered with. 7. Fall <br /> protection: Hand out given. No employee is to wear and or use a harness and or any fall <br /> gear without first inspecting the equipment,up to and including harness, lanyard, and <br /> cable from which you harness to. Inspect for any damages, cuts in the materials, webbing, <br /> and that all labels are inspected. There cannot be any loose threads and or damages which <br /> will render the item useless and in need have replace. Return the item directly to your <br /> (main) supervisor. Your main supervisor shall destroy the item appropriately, cut into <br /> pieces. All safety equipment is of no cost to the employee, however, it is your <br /> responsibility to inspect the equipment before its use. Notify your direct supervisor of any <br /> hazards, if this is not immediately taken care of and or addressed, notify your Safety <br /> Director. Phone numbers are posted. Fall protection shall be worn at all times when <br /> above 6' (feet). 8. First aid boxes, eye wash stations and chemical showers: Inspections <br /> shall occur monthly. This will be performed by staff assigned, and shall be placed out of <br /> order where applicable. Documentation shall be performed and e-mailed to the safety <br /> director. Hard copy shall remain on site. Employees who discover a shower or eye wash <br /> is out of service, need to report the items directly to the manager of the company. <br /> Instructions on how to use the shower and eye wash are given and is hands on. First aid <br /> box: Shall be stocked accordingly. Supplier will arrive monthly to inspect. No pills of any <br /> sort shall be provided. No medications shall be added. If you are discovered adding to the <br /> first aid box, you will be disciplined... 9. Forklift training(REVIEW). Handouts given as <br /> well as a video is reviewed. OSHA poster is posted at facility in multiple locations. 10. <br /> Hearing Program. testing and review of the proper use and safe use of hearing protection. <br /> See handout given as well. <br /> START TIME: 1 hour.Review <br /> END TIME: Instructor: Henry Rotor.Safety Director <br /> DATE: 1-15-19 <br />
