All copies of each receipt shall contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes,the waste stream <br /> type listed in subdivision(c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America(UN/NA) identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address,and identification number; <br /> -The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator,to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain <br /> all required information. <br /> This is a Class II violation. <br /> 121 CCR 66266.81(a)(4)(B) Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. <br /> Bills of lading or manifests for the management of lead acid batteries for the last three years were not available for <br /> review during the inspection.The owner stated that the company that owns the property, Blackmun Equipment <br /> Leasing Co, manages the receipts. A copy of each bill of lading must be kept on site for at least three years. <br /> Immediately locate a copy of all bills of lading or manifests for lead acid batteries for the last three years, maintain <br /> them on site,and submit copies to the EHD. <br /> This is a Class 11 violation. <br /> 125 HSC 25404(e)(4) Failed to report program data in the California Environmental Reporting System (CERS). <br /> A submission to the California Electronic Reporting System (CERS)for the hazardous waste program has not been <br /> made. Beginning January 1,2013, all businesses are required to submit all new(or any changes to existing) <br /> hazardous waste information online to the CERS at hftp://cers.calepa.ca.gov.
