The following is an itemized list of hazardous waste violations that have not been <br /> addressed for ALEXANDERS DIESEL REPAIR as of April29, 2019. <br /> Open violations from March 04, 2019 inspection <br /> Violation #102 - Failed to determine if a waste is a hazardous waste. <br /> A floor scrubber and a mop are used to clean the wash bay and the mechanic bay. All of the waste generated is <br /> dumped down a drain in the wash bay area. <br /> In the outside storage area, one black 55 gallon drum and one 16 gallon drum contained an unknown liquid. <br /> In the hazardous waste storage area, an out of service parts washer contained an oily liquid. The owner stated he <br /> needed to dispose of the liquid. <br /> Any person who generates a waste shall determine if the waste is a hazardous waste. Immediately cease disposal, <br /> make a hazardous waste determination for the wastes, and manage them according to Title 22 hazardous waste <br /> regulations. Submit a statement and supporting documentation explaining how this waste was managed. <br /> Violation #114-Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests for the disposal of oily solids and paper filters were not found on site for 2016 and only <br /> one record was found onsite for 2017 and two for 2018. The owner stated that his waste gets picked up every <br /> couple of months for oily solids/paper filters. Hazardous waste generators shall retain copies of all manifests signed <br /> off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and <br /> have them readily available for review. Immediately locate a copy of all missing manifests and receipts for the last <br /> three years, maintain them on site, and submit all missing copies to the EHD. <br /> Manifest number 01973423JJK was found without a signed off copy from the destination facility. Hazardous waste <br /> generators shall retain copies of all manifests signed off by the destination facility on site for three years and have <br /> them readily available for review. If the generator did not receive a copy of the manifest with the handwritten <br /> signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the <br /> date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing <br /> manifest, with some indication that the generator has not received confirmation of delivery. This information shall <br /> be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifest or prepare and submit the required information to DTSC. Submit <br /> to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. <br /> Violation #115-Failed to comply with uniform hazardous waste manifest exception requirements. <br /> Manifest number 01973423JJK was found without a signed off copy from the destination facility. Hazardous waste <br /> generators shall retain copies of all manifests signed off by the destination facility on site for three years and have <br /> them readily available for review. If the generator did not receive a copy of the manifest with the handwritten <br /> signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the <br /> date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner <br /> or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of <br /> the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, <br /> the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover <br /> letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. <br /> Exception reports shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. Submit to <br /> the EHD a copy of the signed off copy of the manifest or a copy of the exception report submitted to DTSC. <br /> Page 1 of 5 <br />
