Corp, personnel requested an application to the USTCFP and GTI obtained bids to perform the wok approved by the County. 10. On June 1, 1998 Mr. qike Infurna, of SJC PHS/EHD requested changes to the work plan addendum. 11. On July 8, 1998 GTI si ibmitted addendum#2. 12. Mr. Infurna approved E ddendum#2 on September 30, 1998. Copies of the work plan and addendums to the Work Plan, SJC PHS/EHD correspondence, groundwater monitoring r ports, drilling bids and an itemized budget spread sheet of costs for performing this work are a closed for your review. Proposed Direction of Pr dgg The work approved by the County is summarized below: 1. Drill and install three E roundwater monitoring wells to a depth of 30 feet to define the lateral extent of groundwater mpacted by the gasoline. 2. Drill and install one deep soil boring and collect four SimulProbe samples to a depth of 75 feet bgs. Install a shor screened near the depth of 75 feet. 3. Drill and install five-s( it boring to a depth of 20 feet to define the extent of soil impacted by gasoline. One boring i to be converted to a vapor extraction well for use during the remedial phase of work. 4. Perform soil and grour Iwater testing for TPH-G, BTEX, MTBE. 5. Place wells on a quartrly monitoring schedule for one year. Include two monitoring events for testing water samp es for the oxygenated fuel compounds by 8260 and dissolved oxygen and oxidation-reductio i potential. 6. Prepare a site charactei ization report and three monitoring reports. Listed below is a proposed schedule for implementing this project. Proposed Bidding of Proi ect To remain in compliance with the USTCFP, Geological Technics Inc. requested a bid from V&W Drilling to drill an install three groundwater monitoring wells, one deep soil boring and five shallow soil borings. GTI has found V&W to be very cost effective for three primary reasons: • reasonable charge rat • fast drilling equipment • well trained crew A bid from SimulProbe is so included. These costs have been in luded in the attached opinion of cost spreadsheet prepared giving estimated costs for perforri ting the work approved by SCDER. The opinion of cost spreadsheet shows itemized cost for a tasks proposed. In an attempt to make this Pre-Approval flow smoothly, GTI has used same unit costs for this work that has previously been approved by the USTCFP for other prof cts. Geological Technics Inc. accepts field support services and lab invoices and adds a 15% overhead fee. GTI then pa rs these charges. Construction contractors will invoice JM Equipment directly since Geological chnics Inc. is not a licensed contractor.
