On June 1, 1998 Mr. qike Infurna, of SJC PHS/EHD requested changes to the work plan <br /> addendum. <br /> 11. On July 8, 1998 GTI si ibmitted addendum#2. <br /> 12. Mr. Infurna approved E ddendum#2 on September 30, 1998. <br /> Copies of the work plan and addendums to the Work Plan, SJC PHS/EHD correspondence, <br /> groundwater monitoring r ports, drilling bids and an itemized budget spread sheet of costs for <br /> performing this work are a closed for your review. <br /> Proposed Direction of Pr dgg <br /> The work approved by the County is summarized below: <br /> 1. Drill and install three E roundwater monitoring wells to a depth of 30 feet to define the lateral <br /> extent of groundwater mpacted by the gasoline. <br /> 2. Drill and install one deep soil boring and collect four SimulProbe samples to a depth of 75 <br /> feet bgs. Install a shor screened near the depth of 75 feet. <br /> 3. Drill and install five-s( it boring to a depth of 20 feet to define the extent of soil impacted by <br /> gasoline. One boring i to be converted to a vapor extraction well for use during the remedial <br /> phase of work. <br /> 4. Perform soil and grour Iwater testing for TPH-G, BTEX, MTBE. <br /> 5. Place wells on a quartrly monitoring schedule for one year. Include two monitoring events <br /> for testing water samp es for the oxygenated fuel compounds by 8260 and dissolved oxygen <br /> and oxidation-reductio i potential. <br /> 6. Prepare a site charactei ization report and three monitoring reports. <br /> Listed below is a proposed schedule for implementing this project. <br /> Proposed Bidding of Proi ect <br /> To remain in compliance with the USTCFP, Geological Technics Inc. requested a bid from <br /> V&W Drilling to drill an install three groundwater monitoring wells, one deep soil boring and <br /> five shallow soil borings. GTI has found V&W to be very cost effective for three primary <br /> reasons: <br /> • reasonable charge rat <br /> • fast drilling equipment <br /> • well trained crew <br /> A bid from SimulProbe is so included. <br /> These costs have been in luded in the attached opinion of cost spreadsheet prepared giving <br /> estimated costs for perforri ting the work approved by SCDER. The opinion of cost spreadsheet <br /> shows itemized cost for a tasks proposed. In an attempt to make this Pre-Approval flow <br /> smoothly, GTI has used same unit costs for this work that has previously been approved by <br /> the USTCFP for other prof cts. <br /> Geological Technics Inc. accepts field support services and lab invoices and adds a 15% <br /> overhead fee. GTI then pa rs these charges. Construction contractors will invoice JM Equipment <br /> directly since Geological chnics Inc. is not a licensed contractor. <br />
