Piping turbine <br />containment sump was listed as double-walled for all three tanks. This should be listed as single-walled. Riser <br />primary containment was listed as fiberglass for all three tanks. This should be listed as steel. Any change of <br />information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the <br />required information, and submit for review by the EHD. <br />This is a Class II violation. <br />106 <br />CCR 2632 Monitoring and response plans not current or approved by the EHD. <br />The monitoring plan is not current and not approved by the EHD for the 87 main, 91, and 87 siphon in CERS. Tank <br />monitoring must list TLS350 Plus for the monitor panel model and 794390-407 for the leak sensor model for all <br />three tanks. Pipe monitoring and under dispenser containment monitoring must list TLS350 Plus for the monitor <br />panel model for all three tanks. The monitor panel model was only listed as TLS350 for all three tanks. For the 87 <br />main and siphon tanks, nothing was marked in the record keeping section. For the 91 tank, Yes has not been <br />marked for equipment maintenance and calibration records. The record keeping section should be marked yes for <br />the alarm logs and equipment maintenance and calibration records The monitoring plan must be uploaded to the <br />California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, <br />and submit for review by the EHD. <br />This is a minor violation. <br />115 <br />CCR 2715(a) Failed to inform the EHD of a change of designated operator within 30 days. <br />A new designated operator, David Rouse was hired on 10-2-15 and notification was not provided to the EHD within <br />30 days of the change. Any changes shall be reported to the EHD within 30 days.
