Maintenance and monitoring records for the last three years are incomplete. The most recent test results for the monitoring system certification testing, spill container testing, leak detector testing and secondary containment testing were not found on site. The Q2 87 electronic line leak detector alarmed on 12-25-15, 12,15-15, and 12-3-15 for PLLD shutdown alarm. The Q2 87 electronic line leak detector alarmed for gross line fail on 12-15-15. The Q1 91 electronic line leak detector alarmed on 9-20-15 and 8-24-15. The facility and the designated operator did not document these alarms. A service receipt documenting how these alarms were corrected was not found on site during the inspection. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation. 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to document all the alarms from the attached alarm history on the designated operator monthly inspection report and failed to check that they were responded to appropriately. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a minor violation. 208 CCR 2715 Designated operator monthly inspection reports not maintained on site for at least 12 months. The current business operator began operations on 7-30-15. The August 2015 designated operator report was not found on site during the inspection. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the EHD. This is a minor violation.
