The monitoring plan is not current and/or not approved by the Environmental Health Department (EHD). Please make the following changes to the UST Monitoring Plan for underground storage tank TA0186101. Beneath Pipe Monitoring is Performed Using the Following Methods: change the Leak Sensor Model from 8484 to 323; and beneath Training, change "No" to "Yes" for Personnel with UST Monitoring Responsibilities are Familiar with Training Documents. The monitoring plans for underground storage tanks TA0186102 and TA0186103 have not been entered into the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary uploads and changes, and submit for review by the EHD. This is a repeat violation, Class ll. 106 CCR 2632(d)(2) Failed to have an approved UST response plan. The response plan has not been uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a minor violation. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. The secondary containment test results for 5/28/14 were not found on site. The facility failed to document the L2:87 master turbine STP sump sensor out alarm on 4/11/16; the Q1:91 PLLD shutdown and Gross Line Fail on 2/9/16 and 2/17/16 and the Q2:87 PLLD shutdown and Gross Line Fail on 2/22/16 on the daily alarm logs. Records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a repeat violation, Class Il.
