An approved copy of the response plan was not available for inspection. A copy of the plan shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the monitoring system certification, spill container, line leak detector and secondary containment testing on the May through December 2016 designated operator monthly inspection reports. Instead of entering 1/12/16 for the monitoring system certification, spill container and line leak detector testing, the designated operator entered 1/27/16; and instead of entering 5/28/14 for the secondary containment testing, the designated operator entered 3/25/14. The designated operator listed 1/15/15 for the line leak detector and spill container testing on the 4/22/16 designated operator monthly inspection report instead of 1/12/16 for these tests, and listed 1/15/15 for the monitoring system certification, line leak detector, and spill container testing on the 2/26/16 designated operator monthly inspection report instead of 1/12/16 for these tests. The designated operator did not address the Q1:91 PLLD shutdown and Gross Line Fail alarms that occurred on 8/28/16 and 8/29/16 on the 9/20/16 designated operator monthly inspection report. The designated operator did not document the T1:87 Master leak alarm that occurred on 3/14/16 and the sudden loss alarm that occurred on 3/3/16 on the 3/28/16 designated operator monthly inspection report. The designated operator did not complete items 1 through 4 on the designated operator monthly inspection report for 8/18/16 and did not complete items 6, 7, and 8 on the designated operator monthly inspection report for 2/26/16. The designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 211 CCR 2715(f) Designated operator employee training not performed or log not kept. The designated operator employee training for 2016 did not include the Hire Date of the employees The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD.
