Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> #23 Rodents, Insects or Animals Inside Facility MAJOR <br /> OBSERVATIONS:Observed several live cockroaches at the back prep table,wall,wire shelves, and floor. Glue traps are <br /> being used; all traps observed were full of roaches or insects. Immediately take necessary steps to eliminate and/or control <br /> cockroach infestation. Use only food-safe and/or restaurant grade pest control measures. Recommend obtaining <br /> professional pest control service to help control the infestation. <br /> CALCODE DESCRIPTION:Each food facility shall be kept free of vermin:rodents(rats,mice), cockroaches,flies.(114259.1, 114259.4, <br /> 114259.5) <br /> #1 Demonstration of Knowledge <br /> OBSERVATIONS:One person shall obtain the 5 year Food Safety Certificate. Provide a copy to ssangalang@sjcehd.com <br /> within 60 days. <br /> All other employee shall possess the 3 year Food Handler Card. Maintain copies of cards at facility within 30 days. <br /> CALCODE DESCRIPTION:All food employees shall have adequate knowledge of and be trained in food safety as it relates to their <br /> assigned duties.
