SECTION 3- <br /> POTENTIAL HAZARDS <br /> --------------------------------------------------------------------------------------------------------- <br /> Trenches more than four feet deep shall have ladders or steps located <br /> so as to require no more than 25 feet of lateral travel between means <br /> of egress. Ladders shall be placed at an angle not more than 30 <br /> degrees from vertical and secured as necessary. Ladder side rails shall <br /> extend at least three feet above the original ground surface. <br /> Confined Space Entry <br /> Entry into confined spaces is not anticipated. However, if it is necessary, entry will <br /> be performed in accordance with all applicable federal and state regulations. Steps <br /> that will be followed prior to employee entry into a confined space include: <br /> Employee training and indoctrination of confined space entry <br /> procedures prior to performing any such work. This will include <br /> testing of the space prior to entry, use of lifelines, continuous <br /> contamination, and a watch outside the space. <br /> Initial hazard assessment will be compltetd that includes <br /> atmospheric testing of the confined space for, at a minimum, <br /> oxygen content,, flammability, and toxic contaminants. <br /> Mechanical ventilation, as appropriate, will be provided. <br /> The "Inspections and Tests for Entry Into Confined Spaces" form <br /> must be completed and posted prior to entering the space. <br /> Supplied air respirators shall be required for confined space entry <br /> if preentry atmospheric testing indicates contaminant <br /> concentrations greater than two times the Threshold Limit Value <br /> (TLV). <br /> Entry shall be made while wearing a lifeline and harness that is <br /> manned by a standby person. <br />
