The missing alarms <br /> include: <br /> -1_5 (T2 diesel fill 208)fuel alarm on 2/13/18 <br /> -1_18 (T4 used oil fill 208)fuel alarm on 11/29/18 <br /> -The 12/4/18 inspection report stated there were no new alarms. An L18 alarm occurred on 11/29/18. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> The designated operator failed to check and note all required items on the visual inspection reports. <br /> -Overfill prevention, tank tightness, and line tightness test information was left blank on the 10/9/18, 11/6/18, and <br /> 12/4/18 inspection reports. <br /> -Tank tightness and line tightness test information was left blank on the 1/2/19 inspection report. <br /> -Overfill prevention testing was marked as a 'no'on the 1/2/19 inspection report and was not documented in Section III. <br /> -Several items were marked as NA on the 1/2/19 inspection report and were not documented in Section III. <br /> -Section V was left blank on the 10/9/18 and 11/6/18 reports. Neither were signed by the owner/operator. This <br /> section is required to be completed by the site owner or operator within 48 hours of receiving the inspection report. <br /> -The 1/2/19 inspection report did not have a page 2. A copy of the second page was sent over at the time of <br /> inspection. <br /> -The 10/9/18, 11/6/18, 12/4/18, and 1/2/19 inspection reports left blank the question regarding UDC sensor locations. <br /> -The 10/9/18 and 11/6/18 inspection reports did not include a CERS ID. <br /> -The 11/618, 12/4/18, and 1/2/19 inspection reports left blank the question regarding follow from previous inspections <br /> being completed appropriately. <br /> -The 11/6/18 inspection report did not include a secondary containment test date. <br /> -The 11/6/18 inspection report had a blank section III, but marked several items as NA. <br /> -The 12/4/18 inspection report left sections VII and VIII blank. <br /> -The 12/4/18 inspection report did not complete section IX for the used oil and new oil tanks. <br /> During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designated operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> Fuel was observed in UDC 1/2 and several rapid drips were observed on piping and equipment under the dispenser, <br /> indicating a leak in the primary containment. All primary containment for the UST system must be tight.
