San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone: (209)468-3420 Fax; (209)468-3433 Web:www.sjaov.orcI/ehd <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> RYDER TRUCK RENTAL#1071 3633 DUCK CREEK DR STOCKTON January 16 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. <br /> Maintenance and monitoring records for the last three years were not found on site. <br /> -Work orders were not found on site for the L2 alarm on 3/23/18, the L5 alarm on 2/13/18, or the sensor replacements <br /> on 1/5/18. <br /> These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all <br /> monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual <br /> observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other <br /> monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br /> reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on <br /> site and submit copies to the EHD. <br /> This is a repeat violation, Class II. <br /> 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. <br /> An approved, current copy of the monitoring plans were not available for inspection. A copy of these plans shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> This is a repeat violation, Class II. <br /> 206 CCR 2712 Failure to comply with UST operating permit conditions. <br /> The test results submitted on 1/29/18 indicate the used oil fill sump 208 sensor and the new oil fill sump 208 sensor <br /> were replaced on 1/5/18. No notification was submitted to the EHD for these replacements. No technician <br /> certifications were found on site. The following conditions are required to meet No Permit Required requirements: <br /> 1. Email EHD a scope of work by emailing to UST@sjcehd.com. <br /> 2. Notify the EHD of the time and date of the repair work by emailing to UST@sjcehd.com <br /> or fax to (209)468-3433. An EHD inspector may stop by the facility to inspect the repair and verify the technician's <br /> certifications. <br /> 3. The service company must install the part and test the system to verify functionality. <br /> 4. The service company must provide the following documentation to the tank owner/operator for inspector review <br /> during annual UST inspection: <br /> a. Description of part(s) installed <br /> b. Name& UST ICC#of Service technician doing the work (manufacturer certification proof) <br /> c. Testing done to verify functionality of the repair <br /> Immediately obtain a permit for these repairs. A'Failure to Obtain A Permit' penalty may apply. <br /> According to an email from Heather Melton, on May 3, 2018 the main diesel drop tube and spill container were <br /> replaced. At that time, no permit was obtained for these replacements. An permit was obtained from EHD and <br /> inspection was conducted on 5/11/18.
