Failed to keep containers of hazardous waste closed expect when adding/ removing waste <br />and failed to properly label containers. One 55 gallon metal container, cut in half, each stored used oil <br />changed from a compressor placed on top of a secondary containment pallet located in the wooden <br />shed. Neither container had a lid or closure to prevent any contaminates entering into the containers. <br />Neither container had a label to identify what the storage liquid contents. Containers holding hazardous <br />waste must be kept closed during transfer and storage, except when adding or removing hazardous <br />waste and they must be properly labeled with the following information: <br />1. words "Hazardous Waste", 2. name & address of generator, <br />3. hazardous property (toxic, corrosive, ignitable, or reactive) <br />4. physical state (liquid or solid), 5. composition/ contents ie. used oil, 6. accumulation start date <br />Correct by having the contents placed into container with lid and labeled with the above information OR <br />have the contents placed into the 150 gallon used oil tank that is properly labeled. <br />Submit to SJC EHD within 30 days (12/20/12): <br />1. one copy of the completed "Return to Compliance Certification" form <br />2. a written corrective action statement for each of the above violations <br />3. requesting or supporting documentation ie. photos, disposal receipts <br />4. one copy of the completed "California Environmental Reporting System Lead User Authorization <br />Form" to allow EHD authorization to facility's CERS data. Businesses must submit electronic data of <br />HMBP on a state website starting 2013. <br />otes:- <br />Notes.- <br />-Provided: Haz Waste labeling instructions, CERS info & Authorization form, free training schedule <br />- In 2011 year, facility generated approximately 350 gallons of used oil. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: Rece d By: <br />T' I . <br />Thuy Tran (209) 468-8257 <br />EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
