SAN JOAQU I N Environmental Health Department <br /> CO LJ N T Y <br /> Time In: 12:13 pm <br /> Time Out: 12:30 pm <br /> Food Program Complaint Inspection Report <br /> Name of Facility: PIZZA HUT DELIVERY#758525 Date: 03/21/2018 <br /> Address: 720 W ELEVENTH ST, TRACY 95376 <br /> Owner/Operator: PACPIZZA, LLC Telephone: (209)832-5100 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00046015 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> C00046015 states; "COMPLAINANT ALLEGES ON 3/20/2018 AT APPROXIMATELY 7:30 PM THEY WERE AT THE <br /> FACILITY AND OBSERVED THAT IT WAS VERY DIRTY AND AN EMPLOYEE WAS PREPARING PIZZA WITHOUT <br /> WEARING GLOVES." <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> #33 Nonfood Contact Surfaces Clean <br /> OBSERVATIONS:The front counters and cash registers have grease and dust build-up. Clean and sanitize today. <br /> CALCODE DESCRIPTION:All nonfood contact surfaces of utensils and equipment shall be clean.
