Program Element: 1600 - FOOD PROGRAM <br />Telephone: (209) 239-1222 Owner/Operator: KARMA INC <br />Address: 410 EASTWOOD AVE, MANTECA 95336 <br />Date: 03/07/2018Name of Facility: MANTECA CARE & REHABILITATION CTR <br />Food Program Complaint Inspection Report <br />10:25 am <br /> 9:50 am <br />Time Out: <br />Time In: <br />Complaint #: CO0045918 <br />The San Joaquin County Environmental Health Department received the following complaint: <br />5 RESIDENTS AND 1 EMPLOYEE WITH INFLUENZA LIKE ILLNESS, 2 RESIDENTS POSITIVE FOR INFLUENZA B , <br />ONSET 3/1/2018. <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />Environmental Health Department <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />No Temperature Data Collected <br />FOOD ITEM -- LOCATION -- TEMP º F -- COMMENTS <br />NOTES <br />Complaint inspection. Spoke with the administrator, Willy B. DeMesa. <br />Facility is aware of the complaint. They notified Public Health and CA Dept of Licensing. <br />There has been no reported illnesses by employees or family members. <br />The facility keeps a record/log of ill employees. <br />Some employees work for other health care facilities. <br />Staff receives food safety training regularly. <br />Staff does not consume the food at the facility. <br />There has not been any recent (2 weeks) traveling by employees. <br />Mr. DeMesa said that Station 2 has an influenza outbreak. Station 2 consists of 35 residents. 6 of these residents show <br />influenza like illness. 3 have test positive for influenza. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
