1 <br />5.4 Sample Labels <br />Each sample container will be labeled with the following information: <br />- Sample identification number <br />- Date and time of collection <br />- Sample type <br />' - Analyses requested <br />- Initials of sampling personnel <br />15.6 Field Log Book <br />All information pertaining to this field investigation will be recorded in a <br />1 dedicated log book. All entries will be made in ink, and all corrections <br />will be line-out deletions signed and dated. Log book entries will include <br />the following information: <br />1 3542a/BAV102 28 <br />Preceding sampling, labels will be affixed to all sample containers. <br />5.5 Chain -of -Custody Record <br />To establish documentation necessary to trace sample possession <br />from <br />the time of collection to the time of analysis, a chain -of -custody <br />record will accompany every sample shipment to the laboratory. <br />Each chain -of -custody record will contain the following information: <br />- Sampling locations <br />P g <br />- Sampler's signature <br />- Date and time of collection <br />- Sample identification numbers <br />Number <br />- of containers <br />- Name of shipper <br />1 <br />- Date shipped <br />- Signatures of personnel involved in the chain of possession <br />1 <br />Each person in custody of the samples will sign, date, and note the time of <br />change of custody. <br />15.6 Field Log Book <br />All information pertaining to this field investigation will be recorded in a <br />1 dedicated log book.
