Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Upon arrival, some employees were aware of the complaint. <br /> Person in charge states that the facility is sprayed monthly(unsure of the company performing the service). <br /> Upon inspection of the kitchen,warewashing area, dry goods storage area, restrooms, and dining area only one dead <br /> cockroach was observed in the cabinet under the soda dispenser. Observed one glue board/sticky trap on the piping <br /> underneath the hand sink in the kitchen (from Western Exterminators).This glue board was coated in dust/flour and has lost <br /> it's ability to be functional. <br /> Corrective action <br /> 1) Provide copies of the three most recent invoices from your pest control company.
