Copies of hazardous waste disposal records for the last three years were not found on site. <br /> The only disposal records found on site were dated June 6, 2012, for disposal of clean up wastes <br /> associated with the 2011 inspection. Hazardous waste generators shall retain copies of all manifests <br /> signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for <br /> three years and have them readily available for review. Immediately locate a copy of all manifests and <br /> receipts for the last three years, maintain them on site, and submit copies to the EHD by June 19, 2014. <br /> 128. Bills of lading for the recycling of used filters for the last three years were not found on site. A <br /> copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all <br /> bills of lading for used filters for the last three years, maintain them on site, and submit copies to the EHD <br /> by June 19, 2014. <br /> 605. A 5 gallon white plastic bucket containing fuel contaminated waste was observed in the gas station <br /> storage room with no label. According to Ms. Cynthia Perez, the employee on site, she places all of the <br /> contaminated absorbent from spill clean ups in this bucket.
