Since it cannot be determined how long this waste has been on site, immediately contact a <br /> licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the <br /> manifest to the EHD by June 19, 2014. <br /> REPEAT VIOLATION <br /> 812. Copies of universal waste disposal records were not found on site. According to Ms. Cynthia <br /> Perez, the employee on site, all fluorescent tubes changed on site are handled by another employee, <br /> "Alberto" for disposal. A generator of universal waste (batteries, mercury containing equipment, lamps/ <br /> bulbs, electronic devices, CRTs, CRT glass, aerosol cans) shall keep a record of each shipment of <br /> universal waste on site for at least three years. The record may take the form of a log, invoice, manifest, <br /> bill of lading, or other shipping document, and shall include the following: <br /> - Narne & address of destination facility <br /> - Quantity & type of universal waste <br /> - Date of departure/shipment <br /> Begin this practice immediately. <br /> REPEAT VIOLATION <br /> Notice to Comply - Minor violations: (404, 610) <br /> 404. A 5 gallon white plastic bucket containing fuel contaminated waste was observed in the gas <br /> station storage room with no lid. According to Ms. Cynthia Perez, the employee on site, she places all of <br /> the contaminated absorbent from spill clean ups in this bucket. All hazardous waste containers shall be <br /> closed at all times except when adding or removing waste.
