COMPLAINANT <br /> ASKED WAITER FOR A GLASS OF WATER AND WHEN THE WATER WAS RECEIVED THE COMPLAINANT CLAIMS <br /> THAT THEY PUT THEIR STRAW INTO THE CUP AND DRANK WATER WITHOUT LOOKING AT THE CUP.AFTER <br /> DRINKING SOME OF THE WATER, THE COMPLAINANT NOTICED THAT INSIDE THE CUP WAS VERY DIRTY. <br /> COMPLAINANT HAS BEEN VOMITING AND EXPERIENCING STOMACH CRAMPS SINCE THIS INCIDENT. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> D/W-160F <br /> Site visit-Facility was aware of complaint. Observed water and ice,found one black particle in ice storage(removed). <br /> Observed hundreds of empty cups, a few had slight particles(mostly on outside). Some cups have black specks that appear <br /> to be part of the cup. No employees have been sick. Manager states they will retrain staff to check the dishes after they run <br /> through the dishwasher for any particles. Dishwasher is properly running. Dishwasher is lacking a drainage pipe at the end of <br /> the unit.
