Submit a copy of the recordable release statement to the EHD by October 28, 2012. <br />206. The designated operator employee training for Harnik Singh was not performed. Harnik Singh <br />states that he started working at this gas station about two months ago. Harnik Singh was the only <br />employee on site during the inspection. The designated operator shall train facility employees for which <br />he or she is responsible in the proper operation and maintenance of the UST system once every 12 <br />months. The training shall include, but is not limited to - <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD by October 28, 2012. <br />302 & 304. The diesel annular sensor failed to return to all functions normal after the functionality test <br />and, therefore, the tank is not monitored. The interstitial space shall be monitored with a monitoring <br />system connected to an audible and visual alarm system. Immediately contact a properly licensed, <br />trained, and certified contractor to replace the failed component with a functional component (LG 113-x <br />listed, if applicable) under permit and inspection of the EHD within one business day. <br />311. The 87 main tank line leak detector failed to detect a leak when tested. All line leak detectors shall <br />be capable of detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, <br />and certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) and <br />obtain a permit within one business day from the EHD. If the failed leak detector can't be replaced <br />immediately, there is a possibility that the 87 main UST system may be red tagged to prevent fuel inputs. <br />313. The 91 spill container failed when tested. All spill containers shall have a minimum capacity of five <br />gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately <br />contact a properly licensed, trained, and certified contractor to repair or replace the spill container under <br />permit and inspection of the EHD. <br />999. All four mechanical line leak detectors lacked a certification tag/sticker by the technician who <br />performed the July 26, 2011 monitoring system certification testing.
