The tag/sticker shall be placed in a readily visible location and <br />shall include the date the UST component was certified, repaired, or replaced, and the contractor's or <br />tank tester's license number. Correct immediately by taking the necessary precautions to ensure that the <br />tag/sticker with the required information is affixed to the UST monitoring equipment that was certified, <br />repaired, or replaced. <br />306. A fuel leak was found at the diesel filter in dispenser #5/6 and #9/10. All primary containment must <br />be product tight. Immediately clean up any released diesel fuel and have a properly licensed, trained, <br />and certified contractor correct the cause of the leak. Depending on the scope of work a permit and <br />inspection may be required of the EHD. If the failed component can't be replaced immediately, there is a <br />possibility that the diesel UST system may be red tagged to prevent fuel inputs. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by October 28, 2012, <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Aris Cacapit (209) 468-3453 <br />Received By. <br />11� a <br />Title: <br />�'m Lo <br />V i <br />EHD 23-02-003 Rev 07112/11 CONTINUATION FORM <br />
