The diesel line leak detector failed to detect a leak when tested. All line leak detectors shall be <br /> capable of detecting a 3-gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, <br /> and certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) and <br /> obtain a permit within one business day from the EHD. If the failed leak detector can't be replaced <br /> immediately, there is a possibility that the diesel UST system may be red tagged to prevent fuel inputs. <br /> 21. Liquid was observed in the vent box. If water could enter into the secondary containment by <br /> precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this liquid, <br /> make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it <br /> accordingly. Ensure the vent box is maintained free of liquid. <br /> Notes: <br /> -Monitoring plan, approved by this office 7/30/09, is incorrect. Complete and submit a corrected version <br /> to this office by August 26, 2011 for review. Once a approved a copy will be returned to the facility to <br /> maintain on site. <br /> -Diesel annular space sensor sheathing was pulled back when sensor was removed from the interstitial <br /> space. Technician on site pulled and taped the sheathing into the correct position. Inspector will verify <br /> with the manufacturer that this is an approved repair method. If it is not approved by the manufacturer <br /> the sensor will need to be repaired/replaced under permit from this office, according to manufacturer's <br /> instructions. <br /> Complete and submit a copy of the return to compliance certification with a statement addressing how <br /> each of the above listed violations have been corrected to this office within 30 days, August 26, 2011. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURL RATE. <br /> EHD Inspector: Receiv e: <br /> CONTINUATION FORM <br /> EHD 2302-003 v 07/1 1 <br />
