Manifest numbers 011465354,IJ.K (5/15/13), 014058408JJK (7/8/15), and 014058407JJK(7/815)were found without a signed off copy from the destination facility. Manifest numbers 012411721JJK(1/9/14), 0+2421803JJK (3/14/14), 013082578JJK(8/5/14), 013809319JJK(10/16/14), 014051593JJK (3/20/15), and 014058236JJK (4/16/15)were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. This is a Class II violation. 112 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. Copies of hazardous waste disposal records for 2014 and the first half of 2015 were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD. This is a minor violation. 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. Oily staining was observed on the ground around the urea tank and used oil tank inside the warehouse, around the tote fill area inside the warehouse, around the tote storage area in the yard southwest of the building, around the drums on the loading dock, and on the ground in the yard next to the secondary containment of the large tank storage area and the piping connection area. Oily sludge and saturated absorbent socks were observed around a storm drain in the yard; according to Mr. Robinson, the drain leads to the oil-water separator. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately clean all stains, and manage the clean up waste according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed.
