Manifest numbers 012411721JJK(1/9/14), 0+2421803JJK <br /> (3/14/14), 013082578JJK(8/5/14), 013809319JJK(10/16/14), 014051593JJK (3/20/15), and 014058236JJK <br /> (4/16/15)were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the <br /> destination facility on site for three years and have them readily available for review. If the generator did not receive <br /> a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's <br /> waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall <br /> submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation <br /> of delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. <br /> Submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. <br /> This is a Class II violation. <br /> 112 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br /> Copies of hazardous waste disposal records for 2014 and the first half of 2015 were not found on site. Hazardous <br /> waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a <br /> consolidated manifesting procedure on site for three years and have them readily available for review. Immediately <br /> locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the <br /> EHD. <br /> This is a minor violation. <br /> 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br /> Oily staining was observed on the ground around the urea tank and used oil tank inside the warehouse, around the <br /> tote fill area inside the warehouse, around the tote storage area in the yard southwest of the building, around the <br /> drums on the loading dock, and on the ground in the yard next to the secondary containment of the large tank <br /> storage area and the piping connection area. Oily sludge and saturated absorbent socks were observed around a <br /> storm drain in the yard; according to Mr. Robinson, the drain leads to the oil-water separator. Facilities shall be <br /> maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or <br /> surface water which could threaten human health or the environment. Immediately clean all stains, and manage the <br /> clean up waste according to Title 22 hazardous waste regulations.
