CRTs, aerosol cans) shall keep a record of each shipment of universal waste on site for <br /> at least three years. The record may take the form of a log, invoice, manifest, bill of lading, or other <br /> shipping document, and shall include the following: <br /> - Name & address of destination facility, - Quantity & type of universal waste. - Date of departure/ <br /> shipment. Begin this practice immediately and ensure to obtain a record for the shipment of waste <br /> electronic devices on site for a minimum of 3 years. <br /> 60, 61, & 62. Manifest number 006661320JJK shipped 05/06/11 was found without a signed off copy <br /> from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off <br /> by the destination facility on site for three years and have them readily available for review. If the <br /> generator did not receive a copy of the manifest with the handwritten signature of the owner or operator <br /> of the facility to which the generator's waste was submitted within 35 days of the date the waste was <br /> accepted by the initial transporter, the generator shall contact the transporter and/or the owner or <br /> operator of the designated facility to determine the status of the waste. If the generator did not receive a <br /> copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the <br /> initial transporter, the generator shall submit an Exception Report that includes a legible copy of the <br /> missing manifest, and a cover letter signed by the generator explaining the efforts to locate the <br /> hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br /> DTSC Report Repository Generaror Information Services Section <br /> P.O. Box 806 Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. <br /> Submit to EHD a copy of the signed off copy of the manifest or a copy of the exception report submitted <br /> to DTSC. <br /> Submit to SJC EHD within 30 days (08/29/11): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for each of the above violation <br /> Notes: - In 2010 year, facility generated approximately 2885 gallons (11.54 Tons) of used oil, 210 <br /> gallons (.84 Tons) of used antifreeze and .19 Tons manifested waste = 12.57 Tons of hazardous <br /> waste. In months of Jan, Feb, May, June, July, Aug, Nov, Dec, facility generated >1000 kg. <br /> - Provided free training schedule for July - December 2011. Recommended courses: Hazardous waste, <br /> Universal waste and Basic Auto Industry hazardous waste management. <br /> - EHD has been provided a copy of HW tank assessment, re-inspected on 10/2010, for waste oil and <br /> waste antifreeze.
