The record may take the form of a log, invoice, manifest, bill of lading, or other shipping document, and shall include the following: - Name & address of destination facility, - Quantity & type of universal waste. - Date of departure/ shipment. Begin this practice immediately and ensure to obtain a record for the shipment of waste electronic devices on site for a minimum of 3 years. 60, 61, & 62. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository Generaror Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. Submit to EHD a copy of the signed off copy of the manifest or a copy of the exception report submitted to DTSC. Submit to SJC EHD within 30 days (08/29/11): 1. one copy of the completed "Return to Compliance Certification" form 2. a written corrective action statement for each of the above violation Notes: - In 2010 year, facility generated approximately 2885 gallons (11.54 Tons) of used oil, 210 gallons (.84 Tons) of used antifreeze and .19 Tons manifested waste = 12.57 Tons of hazardous waste. In months of Jan, Feb, May, June, July, Aug, Nov, Dec, facility generated >1000 kg. - Provided free training schedule for July - December 2011. Recommended courses: Hazardous waste, Universal waste and Basic Auto Industry hazardous waste management. - EHD has been provided a copy of HW tank assessment, re-inspected on 10/2010, for waste oil and waste antifreeze. In process of being reviewed. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
