Timeln: 1:15pm <br /> Time Out: 1:50 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: CARLS JR RESTAURANT#8750 Date: 11/01/2019 <br /> Address: 6301 W BANNER ST, LODI 95242 <br /> Owner/Operator: Telephone: (209)366-2206 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00050971 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> BATHROOMS HAVE BAD SMELL LIKE SEWAGE BACKED UP. NO TOILET PAPER AND TRASH CANS ARE <br /> OVERFLOWING. SODA MACHINES ARE DIRTY AND HAS MOLD AROUND IT. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Spoke to store manager who was aware of this customer. She stated that he complained to her and at time of complaint, <br /> there was nothing wrong, except that some customers threw paper towels on the floor. <br /> Observations: Inspected both rest rooms. There is slight sewer odor in rest rooms.
