S ■ J O A Q U I ` , Environmental Health Department <br /> N <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Dollar Tree#03771 7528 Pacific Ave Stockton A ril 23 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> -An empty container of The Home Store Glass Cleaner was found without an indication that the item was for store <br /> use, and without absorbent to account for the missing liquid(IMG 5515-6).Albert stated he did not not know what <br /> happened to the missing liquid. <br /> -A partially empty container of Baby Love Baby Oil was found without an indication that the item was for store use, <br /> and without absorbent to account for the missing liquid(IMG 5517-8). Albert stated he did not not know what <br /> happened to the missing liquid. <br /> -An almost empty container of Body Wash was found without an indication that the item was for store use, and with <br /> minimal absorbent, but not enough absorbent to account for the amount of missing liquid(IMG 5519-20). Albert stated <br /> he did not not know what happened to the missing liquid. <br /> -An empty container of Jot Clear Glue was found without an indication that the item was for store use, and without <br /> absorbent to account for the missing liquid(IMG 5521-2).Albert stated he did not not know what happened to the <br /> missing liquid. <br /> -The following HW items were found an STOU HW tote(IMG 5523): <br /> -An almost empty container of Spa Naturals Bubble Bath was found without an indication that the item was for store <br /> use, and without absorbent to account for the missing liquid(IMG 5524). Albert stated he did not not know what <br /> happened to the missing liquid. <br /> -An empty container of Suavitel fabric softener was found without an indication that the item was for store use, and <br /> without absorbent to account for the missing liquid(IMG 5525).Albert stated he did not not know what happened to <br /> the missing liquid. <br /> The following was observed near the front of the facility: <br /> -A leaking Bolero Peonies&Pear Body Oil was found leaking in a HW bag in an open Dollar Tree basket labeled as <br /> "Damaged Goods"(IMG 5530, 5532-4).The hand-held scanner indicated the item was not hazardous(IMG 5536), <br /> however Albert stated, although he did not know who placed the item in the HW bag, he would have also classified the <br /> item as HW.Albert placed the item into an STOU HW tote in the HW storage area. <br /> -An unlabeled tote was found on a shelf,which Albert stated was used to temporarily used to store HW(IMG 5530). <br /> Albert stated when store employees find a leaking or damaged item that can no longer be sold, the employee will <br /> place the item into a HW bag located behind register one(IMG 5531), and place the item into the unlabeled tote. <br /> Albert stated the items in the tote are sorted into their appropriate HW tote by a manager at least once a day.There <br /> were no items in the unlabeled tote at the time of inspection, however the inspector notified Albert that once an item is <br /> determined to be HW, it must follow all of the applicable HW regulations, including proper labeling, and proper <br /> management of incompatible wastes.Albert understood, removed the unlabeled tote from the shelf, and indicated he <br /> would inform the employees that any HW items found are to be processed into a HW tote immediately. <br /> Albert provided the inspector with a record of hazardous waste training for the current facility employees. Based on <br /> this record, all current employees were provided"HAZMAT Handling and Processing"training,except for Julia <br /> Chesley. <br /> The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste <br /> handling and emergency procedures. <br /> Provide proof of re-training for all current facility employees and initial training for Julia Chesley, and training materials <br /> FA0018691 PR0538092 SCO01 04/23/2019 <br /> EHD 22-01 Rev.09/27/2018 Page 6 of 9 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
