A UW handler shall label or mark UW to identify the type of waste. <br />Lamps (i.e., each lamp), or a container in which the lamps are contained, shall be labeled or marked <br />clearly with the following phrase: "Universal Waste-Lamp(s)". Correct immediately by labeling the UW <br />lamps as "Universal Waste Lamps". Class II Repeat Violation. <br />84. The facility did not demonstrate when the UW lamps became a UW. A UW handler shall accumulate <br />UW for no longer than one year from the date the UW was generated, or was received from another UW <br />handler. A UW handler shall be able to demonstrate the length of time that the UW has been <br />accumulated from the date it became a waste or was received. The UW handler may make this <br />demonstration by: <br />(1) Placing the UW in a container and marking or labeling the container with the earliest date that any <br />UW in the container became a waste or was received. <br />(2) Marking or labeling the individual item of UW with the date it became a waste or was received. <br />(3) Maintaining an inventory system on site that identifies the date the UW being accumulated became a <br />waste or was received. <br />(4) Maintaining an inventory system on site that identifies the earliest date that any UW in a group of <br />items of UW or group of containers of UW became a waste or was received; <br />(5) Placing the UW in a specific accumulation area and marking or labeling the area to identify the <br />earliest date that any UW in the area became a waste or was received; or <br />(6) Any other method which clearly demonstrates the length of time that the UW has been accumulated <br />from the date it became a waste or was received. <br />Employee acknowledged that the UW needs to be disposed within one year of the accumulation start <br />date. Since it cannot be determined how long this waste has been on site immediately dispose of the <br />UW lamps properly. Submit a copy of the disposal receipt to this office by 7-22-11. Class II Repeat <br />Violation. <br />85. Employee stated that the two UW electronic computer monitors that were noted in the 3-16-10 <br />routine hazardous waste inspection have been disposed of. The disposal receipt for these UW electronic <br />computer monitors is not on site. A UW handler shall keep a record of each shipment of UW sent from <br />the UW handler's facility to another facility. The record may take the form of a log, invoice, manifest, bill <br />of lading or other shipping document. The record for each shipment of UW sent shall include the <br />following information: <br />(1) The name and address of the UW handler or destination facility to which the UW was sent; <br />(2) The quantity [count or weight, consistent with, for example, section 66273.32, subsection (d)] of each <br />type of UW sent (e.g., batteries, thermostats, lamps, electronic devices, CRTs, CRT glass),- <br />lass);ALL <br />ALLEHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />EHD Inspecto 7 <br />Received <br />Z 1&�_, <br />Title: <br />EHD 23-02-003 Rev 08/10/10 `� J CONTINUATION FORM <br />
