SAN10 A Q U I N Environmental Health Department <br /> C l_J U '1�j I Y <br /> Complaint Record Daily Time and Activity Summary from 4/19/2017 to <br /> 4/19/2019 for C00046832 located at <br /> 6440 AVIATION DR <br /> Activity Date Recorded by <br /> 06/04/2018 10:18 pm- 10:38 pm, received a page, called General Hospital concerning PE 2546 MANZO <br /> 30 min transformer spill. Approximately 15 gallons of transformer oil spilled. Called Tasha SC 978 <br /> Shasta (with PGE) 510-334-6725, she reported that a release occurred from a hit <br /> pad mounted transformer, approximately 15 gallons of non-PCB transformer oil <br /> was released onto dirt. An oil sample will be collected tomorrow for PCB content. <br /> Dave Larsen with PGE 916-897-6779 will be doing the clean up tonight, the <br /> transformer will be replaced tonight as well. Tasha indicated that she would prefer <br /> I wouldn't call Dave Larsen and to communicate through her, since she thinks she <br /> wasn't supposed to provide his number to me. I told her that I believe I already <br /> have his number programmed into our ER phone and if so I will go ahead and call <br /> him to get more info on ETA. Called Dave on both numbers programmed on ER <br /> phone, left a message on 209-401-9719 number for Dave, tried calling multiple <br /> times, he never picked up. Called Tasha back asking her for the ETA. She <br /> indicated that they will replace the transformer and excavation will start at 2 am <br /> and she thinks if I arrive at 3 am I will be able to verify clean up. She asked to be <br /> called and updated and to not call Dave. Documented the complaint. <br /> 06/05/2018 Received spill information from Elena Manzo. Accident occurred at night. Elena PE 2546 ALANIZ <br /> 90 min went out to witness clean up but was unable to locate it. Called Alex Steele at SC 544 <br /> 8:50 a.m. He said that Dave Larsen would be out at the site at 10:30 a.m. to verify <br /> that the site was cleaned up properly. Discussed with Elena Manzo. Left for site <br /> at 10:15 to gas, and arrived at 10:45 a.m. Met with Dave Larson. He explained to <br /> me that a farmer had hit the transformer with equipment and spilled out 6 gallons <br /> of oil. The transformer was put into place approximately three years ago and is a <br /> blue sticker. Four 55 gallon drums were filled in the clean-up process. A sample <br /> is going to be tested for PCBs. As of now, the drums are labeled as hazardous <br /> waste and are pending analysis. <br /> The site has been properly cleaned-up.
